If you would like to be added to the waiting list please complete this form.  Families will be notified of openings based on availability.  Thank you for your interest in the Horizon co-op.

You will need to renew your information to the waiting list on or after April 1st, each year you are requesting membership.  Once you have added your student to the list, we will keep your information on file for the current school year.  (Our school year runs from September-May).

Also, please be advised that as a part of our registration you will be asked to submit a background check from First Baptist Church of Champaign in Savoy. This is a federal background check that must include your SSN.
